Transom Drain Problem

This winter I need to fix the drain in the sump back by the transom (if I said that right). I have a 1978 13' Whaler and metal sleeve is about worn out - the flange located at the inboard end is breaking apart to where chunks are missing and the hole is so loose that the plug sometimes pops out. Plus I am getting an increasing amount of leakage around the outside sleeve which must be bad bad bad.
I bought a replacement sleeve but cannot get the old one out. I am imaging having to somehow drill or hack it out, then back fill the wound with some kind of hardening material, and then slip in the new sleeve.
Am I on the right track?
